Second Semester Commitment Form

Second Semester Commitment Forms are due Dec. 11 at 8 a.m. Parents must complete a Second Semester Commitment Form for for ALL PreK-12 students, even if their child is not changing learning options. Any requests for changes made after this date will not be honored. Parents must submit one form for each child in their household.

This commitment is for the entire semester (January-May).

There are three options: In-Person, Virtual or Self-Paced.

  • In-Person: Learning occurs daily at school.
  • Virtual: Learning occurs at home, and students are online from 8:00 a.m. - 3:00 p.m.
  • Self-Paced: Program-driven option that allows students to complete the work anytime (i.e. evenings, weekends). Teachers are assigned to the students and will monitor progress. Students should expect to complete 35+ hours of work per week.

At the end of the form, there is the option to add additional students.
